How Epalrestat works
Epalrestat works by inhibiting an enzyme (aldose reductase) which converts glucose into sorbitol. In diabetics, there is excess production of sorbitol that gets deposited in nerves and other tissues, causing diabetic neuropathy. Thus, by inhibiting the enzyme, Epalrestat delays sorbitol accumulation in nerves and other tissues thereby delaying the progression and occurrence of diabetic neuropathy in affected individuals.

- signs of elevated liver enzymes such as dark urine, tiredness, itching, jaundice (yellowing of your skin or eyes), and loss of appetite
